How does KeepMEWell.org work?
KeepMEWell.org is an online health check that begins with your own personal health assessment. By simply answering a few questions, KeepMEWell.org will give you a personal health report that will help you learn if you are at risk for chronic diseases.
Based on your answers to a series of questions, you will get the following three reports:
· Scorecard: A summary of your risk for chronic diseases.
· My Report: Information on how you can reduce possible risks along with links to trusted health information websites.
· Local Community Supports and Programs: A list of local community supports and programs that can help you improve your health.
The assessment is for Mainers age 18 years and older. It takes about 10 minutes to complete and your answers and results are anonymous. No one will know who you are and your information will not be shared with anyone. No personal information, such as your name or phone number, will even be collected.
